*Must not have hospitalizations for psychiatric care in the last 6 months

How the CMA makes the appointment: 1. Department: MWC, PHC or WCHC FAMILY PRACTICE2. Visit type: "pain"3. If a pop-up box "Provider Resource Selection" appears, close it4. Provider: "all" 6.
